Request a referral from your specialist or primary care physician.
Recall and write down any questions or concerns related to your medical problems you would like to discuss with our physicians.
As a new patient, please bring the following items to your first appointment:
A copy of your driver’s license or state-issued identification card
Your insurance information
All medications you are currently taking
A copy of your medical records, lab results, x-ray and CT films, disks or reports if available.
Be prepared to make your co-payment if necessary.
Answer all questions you are asked about your condition and medical history to the best of your ability.
Ask any questions you may have about your condition and treatment plan.
Follow up with any plans that were discussed during your appointment, such as x-rays, laboratory tests, allergy testing, CT scans, sleep studies and follow-up appointments.
Pick up medications from your pharmacy prescribed to you by your physician.
